Care Coordination services for children in Suffolk County.
Integrated Mental Health Workers coordinate care including Skill Building and Youth Peer Advocacy for children and Family Peer Support for parents/guardians. Youth Advocacy services are designed to empower youth through a connection with someone who understands what it is like to receive services.
Family Peer Advocates assists to empower parents/guardians based on a unique qualification of having a similar parenting experience. They have first-hand knowledge of services and can connect families with similar experiences for increased support and social networking.
