CFTSS
Children and Family Treatment and Support Services are intended to better meet children’s needs, expand access to clinical treatment services, and provide a greater array of approaches for rehabilitative interventions by providing services in home or in the community. Services are provided to those ages 5-21 with active Medicaid.
Community Psychiatric Supports and Treatment (CPST): Trained staff help incorporate your child’s therapy and treatment goals into their everyday lives. Focus is on building and improving relationships, fostering communication, identifying coping skills and managing emotions.
Family Peer Support Services (FPSS): Family Support Services are provided by credentialed Family Peer Advocates who work specifically with you as the child’s parent. Family Peer Advocates are parents who are raising or have raised a child with a mental health diagnosis and are personally familiar with the associated challenges. Advocates have first-hand knowledge of the services and supports available in the community and will empower you to advocate for these services. There are monthly activities such as support groups, workshops, family engagement events, etc. to provide you with a forum to engage other parents with similar experiences.
Other Licensed Practitioner (OLP): Provides assessments, therapy and treatment planning for mental health needs. It is provided in the home and/or community by a licensed clinician.
Psychosocial Rehabilitation (PSR): This includes hands on and task orientated activities to help restore and rehabilitate your child’s social and interpersonal skills, daily living skills and community integration.
Family Center Nassau
Respite Program-Respite services are designed to support families, reduce stress and help prevent long-term hospitalizations or higher levels of care. Respite services provide a break for the family and decreases stress at home to promote overall wellness for the family, and consists of 26 sessions. Respite services youth ages 5-18 with an SED diagnosis who reside in Nassau County.
Family Support Services – Families are assigned a Family Peer Advocate (FPA) who helps navigate services and provides support based on shared living experience. The goal of FSS is to empower families and reduce the need for higher levels of care. Family Support Services serves families raising children ages 5-21 who either have an SED diagnosis or are at risk for developing one and reside in Nassau County.
Health Home Care Management
FCA’s Health Home Care Management program provides comprehensive service coordination to those with a mental health diagnosis or multiple chronic conditions by ensuring the individual has access to appropriate services to help improve their health outcomes and prevent inpatient and emergency room visits. This coordination can include linkage to needed supports and resources within the community and collaborating with other treatment providers.
Services are provided to those ages 5 through adulthood with active Medicaid. The program serves both Nassau and Suffolk residents.
Non Medicaid Care Management
FCA’s Non-Medicaid Care Management program provides the same level of service coordination as the Health Home program. Services are provided to those ages 5 through up to their 21st birthday who have a mental health diagnosis and reside in Nassau County.
Youth and Family Integration (Y-Fi) Program
FCA’s Youth and Family Integration program provides the same level of care coordination as the other care management programs in addition to skill building and family support services to youth who have or are at risk of developing a mental health diagnosis. Services are provided to those ages 5 through up to their 21st birthday who reside in Suffolk County.
